Documents must match the customer profile via name, address, etc. versus an account that you create on our website. Acceptable Forms of Proof of Military Service
- DD Form 214 (Certificate of Release or Discharge): The most common document for veterans. It shows dates of service and discharge characterization.
- Military ID Cards: Active Duty, Reserve, or Retired ID cards (DD Form 2 or similar).
- Driver’s License/State ID with Veteran Designation: Many states now print "Veteran" or a star on licenses after verifying a DD-214.
- VA Identification Card: Specifically, the Veteran Health Identification Card (VHIC) or the Veteran Identification Card (VIC).
- Leave and Earnings Statement (LES): A recent pay stub for active duty members.
- NGB Form 22: For National Guard members.
